- Strength: Difficulty lifting or carrying 10 pounds
- Assistance with walking: Difficulty walking across a room
- Rise from a chair: Difficulty getting up from a chair or bed
- Climb stairs: Difficulty climbing 10 stairs
- Falls: Number of falls during the past year
Each area receives a score from 0 to 2. The total score ranges from 0 to 10.
